Request of Transcripts

Please follow this process to request a transcript or call the school at 615-865-4055 Ext 6008 for the registrar.

1. Click the button below to request a transcript.

2. Payment: Once you submit the questionnaire you will be asked to submit a $5 payment through our donation platform link. Enter the amount and select Transcript Request at the bottom of the drop-down donation menu.
